In de nabije toekomst heeft Amerika de strijd tegen drugs verloren en laten geheimagenten naast hun identiteit ook hun gezicht veranderen. Fred (Keanu Reeves) is zo’n undercoveragent, die een probleem krijgt wanneer hij teveel drugs (het populaire Substance D.) gebruikt, waardoor hij schizofreen wordt. Hierdoor ontstaat Fred's alter ego Bob, een meedogenloze drugsdealer.
