Walter Sparrow krijgt voor zijn verjaardag een boek, getiteld 'The Number 23'. Wat eerst een onschuldig cadeau lijkt, verandert al snel in een regelrechte nachtmerrie. Hoe meer Walter leest, hoe meer gelijkenissen hij ontdekt met zijn eigen leven. Hij wordt als het ware achtervolgd door het cijfer 23.
