Als een stad getroffen wordt door een mysterieus virus dat alle inwoners blind maakt, besluiten de autoriteiten het gebied af te sluiten. Er is echter een vrouw (Julianne Moore) die nog niet getroffen is door het virus, en aangezien alleen zij nog kan zien, rust er een enorme verantwoordelijkheid op haar schouders.
