Il se fait connaître au Danemark pour sa participation aux films Pusher (1996) et Pusher 2 : Du sang sur les mains (2004), tous deux réalisés par Nicolas Winding Refn et pour la série Rejseholdet (2000–2004). Il est rendu célèbre à l'international en interprétant Le Chiffre, l'un des ennemis emblématiques de James Bond, dans le film Casino Royale (2006). Depuis, il s'est fait une place dans le paysage cinématographique international, interprétant notamment le Dr Hannibal Lecter dans la série Hannibal (2013-2015) de Bryan Fuller, Gallen Erso dans Rogue One: A Star Wars Story (2016) ou encore Kaecilius, antagoniste du film Doctor Strange (2016). En parallèle, il continue également de tourner des films danois et européens, apparaissant notamment dans Le Guerrier silencieux (2009), La Chasse (2012), pour lequel il remporte le prix d'interprétation masculine au festival de Cannes, Royal Affair (2012), ou encore Michael Kohlhaas (2013).
